Halo 3 Mishaps - lots of 'em
A discussion a couple of days ago over on the NeoGAF forums turned up a few interesting 'mishap' vids created by MiniWaz - they're not that new, but they're a lot of fun. Some lag, some silly mistakes - good watching. You can find a large two-parter on YouTube (Part 1 | Part 2), and a smaller (just under 5 minutes) montage on GameVee. Always fun to watch WTF moments!
